Step 1
A referral comes from a care manager, health plan, or the member.
Step 2
The member meets a Specialist or licensed clinician for an assessment and a person-centered plan with named goals.
Step 3
A Relational Skills Specialist works with the member toward those goals, in person or remotely, with ongoing clinical supervision.
